Skip the flu with three simple steps:
1) Fill this Google registration form out (click here). That will tell us who’s coming and provide CHI SJH with important information to have enough vaccines.
2) Print and fill out the attached consent form, and bring it with you (a Spanish version will be provided as soon as it is available).
3) Show up (Be brave! A tiny little needle beats the big bad flu, every time).
