If you’d like to CHANGE your child’s Instructional Model (At-Home, On-Campus, Hybrid 7-12) for the 3rd Six Weeks of school you must complete this form by Monday, October 19 at 11:59 p.m. If you are not making a change, then you do not need to do anything. Please complete a form for each child that you would like to change for the 3rd Six Weeks.
