If you plan on participating in sports (including cheerleading) next school year, you will need to have a pre-participation physical completed before the first day of school and complete a series of online forms. As of today, all current 6th graders have been enrolled into the BISD Rank One system where the online forms are located. The system will show that all Rayburn students are going to Davila Middle School. Go ahead and complete forms. Once the school year has ended, the system will roll over and should update to the correct campus if your student is attending SFA next year.
- The pre-participation physical and the online forms must be dated after April 1st and completed before the first day of school in August.

- Page 1 is filled out by the parent/guardian and signed before you go to the doctor. Page 2 is completed by your doctor.
- You will turn in your printed and completed physical form to your new middle school coaches. Do not bring to Rayburn.
- Any students with a history of asthma, diabetes, seizures or severe allergies requiring an EpiPen will need to have their doctor complete an action plan along with the physical. The action plans can be downloaded from our district athletic website. Click here and scroll to the bottom.
